Our health is affected by things we can change and things we can't . Our lifestyle - what we eat, how much we drink and smoke, how much physical activity we get and the way we feel about ourselves has a big impact on our health.

Surprisingly some small changes can make a big difference, for example if you are overweight, losing only a small amount of weight can significantly reduce your chances of contracting diabetes and cardio vascular disease. If you are feeling low in mood, building regular physical activity into your daily routine can turn this around.

Community Health Trainers (CHTs) work with you on a one to one basis to look at the things you can change to identify what may affect your health. They then work with you to create a realistic Health Action Plan based on your priorities and needs. Your Personal Health Action Plan will include any support you may need to make these changes or referral to specialist services such as the Greenwich Stop Smoking Service.

CHTs are drawn from various communities around Greenwich and are either volunteers or frontline workers from a range of organisations. They live and work in the same communities as you and will support you through those difficult first steps.

They are fully trained by NHS Greeenwich to work with people to identify lifestyle issues that could be affecting their health, motivate people to make the first steps to change their lifestyle and then point them in the direction of services which will provide the long term support and information people need to make these changes. They work one to one and are available for up to six one hour sessions. They also work closely with community centres and GP practices.

Another aspect of a CHT's work is to identify barriers that people may encounter when accessing services. They then work with all parties to remove these obstacles where possible. This may be something simple such as a clinics opening hours or more complex problems such as accessing services across borough boundaries. The result is better access for all service users.
